    Accidents and incidents can happen at any time despite all the precautions. These unplanned or undesired events can have a negative impact on an organization's operations, including work-related injuries, occupational illnesses, property damage, spills, and fires. It is critical that all incidents be thoroughly investigated using a proven company-wide process that is efficient and effective and linked to the Safety Management System (SMS). The criticality of the incident will define the extent of the investigation.

    However, even minor incidents require a thorough investigation, as they may reveal significant potential for a serious future incident.  Velosi’s Health & Safety training course on Advanced Incident Investigation will set forth the confidence and professionalism necessary to conduct a thorough incident investigation.

  • By the end of this course, participants will learn to:

    • Define accidents and their relationship to incidents.
    • Utilize accident causation models to explain the causes of accidents.
    • Interview witnesses and conduct initial actions following an incident on site, including collecting factual evidence.
    • Employ effective methods for evidence gathering and preservation.
    • Lead an Incident Investigation Team and coordinate its activities.
    • Generate a corrective action report and presentation.
    • Conduct root cause analysis using modern incident investigation and analysis techniques, including Cause Tree Analysis (CTA), Fault Tree Analysis (FTA), and Events and causal Factors Analysis (E&CFA).
    • Utilize incident investigations as a management audit tool to identify and address active human error, preconditions, and root causes.
    • Plan and implement action plans for reactive and proactive monitoring of performance, including benchmarking within the industry.
    • Communicate the Safety Management System (SMS) and the company’s safety culture.
    • Comply with legal requirements and adhere to company procedures.
  • Training will be conducted via Microsoft Team Meeting. Meeting invites will be shared one day before the first day of training. 

    Face-to-face training is available as per request.
