  • By the end of this training, participants will be able to:

    • Understand the purpose and importance of incident investigations.
    • Identify different types of incidents (near misses, minor, major).
    • Apply a structured methodology for conducting investigations.
    • Collect, document, and analyze evidence effectively.
    • Perform root cause analysis using tools such as the “5 Whys” and Fishbone Diagram.
    • Develop corrective and preventive action plans to prevent recurrence.
    • Prepare clear, concise, and comprehensive investigation reports.
    • Understand legal and regulatory requirements related to incident reporting.
