VAIL-PHA (Process Hazard Analysis)

VAIL-PHA (Process Hazard Analysis)

Process Hazard Analysis (PHA) is a key element of every successful Process Safety Management (PSM) program. The purpose of such an analysis is to identify hazards within the subject process. This is achieved by systematically reviewing the process and subsequently eliminating or controlling conditions that could lead to hazardous events.

A PHA provides information intended to assist managers and employees in making decisions for improving safety and reducing the consequences of unwanted or unplanned releases of hazardous chemicals.

A PHA is directed toward analyzing potential causes and consequences of fires, explosions, releases of toxic or flammable chemicals and major spills of hazardous chemicals, and it focuses on equipment, instrumentation, utilities, human actions, and external factors that might impact the process.

A PHA is a form of analysis that can take many forms, from qualitative to semi-quantitative to fully quantitative. Examples of PHAs include:

  • Hazard and Operability (HAZOP)
  • Hazid identification (HAZID)
  • Layers of Protection Analysis (LOPA)
  • Event Tree Analysis
  • Fault Tree Analysis
  • Bow-Tie Analysis
  • What-If and Structured What-If (SWIF)
  • Failure Mode and Effects Analysis (FMEA)
  • Quantitative Risk Assessment (QRA)

OSHA requires employers to perform a PHA on each process covered by the law. The purpose of the PHA is to identify and evaluate the hazards of the process, and ways or methods to control them.

Velosi’s experienced staff can assist clients in selecting and implementing the right method to evaluate activities as varied as a traditional process, offshore/onshore production, pipeline, instrumented and electrical distribution systems, as well as specific activities such as written procedures, crane operations, among many others.
